• Consent: By using the YodocEMR™ platform, you acknowledge the potential risks and agree not to hold Health Lync LLC responsible for any breaches or unauthorized access to your PHI.

FINANCIAL RESPONSIBILITY AGREEMENT

I understand and agree to the following financial terms related to my use of the YodocEMR™ platform provided by Health Lync LLC:

• Billing: I understand that I may be charged for telehealth consultations and other services provided through the YodocEMR™ platform as agreed upon with my healthcare provider.
• Insurance: I acknowledge that it is my responsibility to verify if my insurance covers telehealth services and to provide accurate insurance information to ensure accurate billing.
• Payment: I agree to pay any applicable fees and charges associated with the services provided through the YodocEMR™ platform in a timely manner as agreed upon with my healthcare provider.

PATIENT AGREEMENT AND ACKNOWLEDGMENT OF RISKS

1. Platform Risks: I understand that the YodocEMR™ platform uses OpenEMR and a free Zoom plugin for telehealth services. While Health Lync LLC takes measures to secure patient data and meet HIPAA requirements, I acknowledge that the use of third-party components introduces potential risks beyond the control of Health Lync LLC.
2. Data Transmission: I understand that electronic transmission of patient data, including personal and medical information, carries inherent risks. Health Lync LLC uses SSL encryption for data transmission and storage per HIPAA laws, but I acknowledge that no system can be guaranteed to be completely secure.
3. Indemnification: I agree not to hold Health Lync LLC responsible for any breaches of security or unauthorized access to my patient data that may occur while using the YodocEMR™ platform. I understand that Health Lync LLC is not liable for any damages or losses resulting from the use of the platform.

• Adhere to all terms, conditions, and guidelines outlined by Health Lync.
• Consult only for non-emergency situations and follow the advice provided by professionals.

4. Timely Communication

• Be responsive and timely in communication with healthcare providers.
• Provide accurate and complete information as required.

5. Responsible Usage

• Use the platform only for its intended purpose, avoiding any fraudulent or misleading activities.
• Report any suspicious activities or ethical concerns to Health Lync immediately.

1. Professionalism and Ethical Practice

• Follow accepted medical guidelines, standards, and practices.

2. Confidentiality

• Maintain the confidentiality of patient information in compliance with applicable laws.
• Ensure secure handling and storage of patient records.

3. Respect for Patient Rights

• Respect the autonomy, privacy, and dignity of patients.
• Provide clear and comprehensive information, ensuring patients' informed consent.

4. Responsiveness and Communication

• Respond to patient inquiries in a timely and professional manner.
• Communicate clearly, avoiding medical jargon when possible, and ensure patients understand their care and treatment options.

5. Compliance with Health Lync Policies

• Adhere to all rules, terms, and conditions provided by Health Lync.
• Avoid advertising, solicitation, or any unprofessional behavior on the platform.

6. Continuous Learning

• Commit to continuous professional development, staying up-to-date with the latest medical knowledge and techniques.

7. Reporting and Accountability

• Report any unprofessional or suspicious activities by patients or other doctors to Health Lync.
• Accept responsibility for the accuracy and quality of the medical advice provided.
